    1. Agreement to terms

    By accessing or using the Forward Health platform at getforwardhealth.com ("Service"), you agree to be bound by these Terms of Service ("Terms"). The Service is operated by Growth Engineer LLC ("Company," "we," "us," or "our"), a company registered in Washington, D.C.

    If you do not agree to these Terms, do not use the Service.

    2. Description of service

    Forward Health provides a telehealth platform that connects patients with licensed, board-certified physicians for disability parking permit evaluations. Upon completing your intake and evaluation, you will receive physician-signed certification forms required by your state's DMV to apply for a disability parking permit.

    We do not issue disability parking permits or placards. Permits are issued solely by your state DMV upon review of the physician-signed documentation.

    3. Eligibility

    You must be at least 18 years old to use the Service. By using the Service, you represent that you are at least 18 years of age and have the legal capacity to enter into these Terms.

    You may use the Service to obtain an evaluation for yourself or, as a legal guardian, on behalf of a minor or dependent in your care. You are responsible for ensuring all information submitted is accurate and truthful.

    4. Telehealth services and physician-patient relationship

    The evaluations provided through our platform constitute a telehealth encounter between you and a licensed physician. The physician-patient relationship is limited to the scope of the disability parking permit evaluation. This service is not a substitute for ongoing primary care, emergency services, or comprehensive medical care.

    5. Payment and refund policy

    The current fee for a disability parking permit evaluation is $129, subject to change with notice. Updated pricing will be reflected on the website prior to any charge. Payment is due at the time of scheduling. We accept major credit and debit cards via our secure payment processor.

    If you complete your evaluation and the physician determines that you do not meet the medical criteria for a disability parking permit, you will receive a full refund. For full refund terms, see our Refund Policy.

    6. Accuracy of information

    You agree to provide accurate, complete, and truthful information in your medical intake. Submitting false or misleading medical information to obtain a disability parking permit may constitute fraud under applicable state and federal law. We reserve the right to terminate your access to the Service if we have reason to believe information you submitted was intentionally false.

    Following termination of access for submission of fraudulent information, your medical intake records are retained in accordance with applicable healthcare recordkeeping requirements and will not be used for any purpose beyond legal compliance.     7. Limitation of liability

    To the fullest extent permitted by applicable law, Growth Engineer LLC and its officers, employees, agents, and contractors shall not be liable for any indirect, incidental, special, consequential, or punitive damages arising out of your use of the Service. Our total liability for any claim arising from or related to the Service shall not exceed the amount paid by you for the specific evaluation giving rise to the claim.

    8. Disclaimer of warranties

    The Service is provided "as is" and "as available" without warranties of any kind, either express or implied. We do not warrant that the Service will be uninterrupted, error-free, or that defects will be corrected. We do not guarantee that your DMV will approve your permit application; DMV approval is at the sole discretion of the DMV.

    9. Governing law and dispute resolution

    These Terms are governed by and construed in accordance with the laws of the District of Columbia, without regard to its conflict of law provisions. Any dispute arising from or related to these Terms or the Service shall be resolved by binding arbitration in Washington, D.C., except that either party may seek injunctive relief in a court of competent jurisdiction.

    10. Changes to these terms

    We reserve the right to modify these Terms at any time. We will provide notice of material changes by updating the "Last updated" date at the top of this page. Your continued use of the Service after changes are posted constitutes your acceptance of the revised Terms.
