About Forward Health

Forward Health is a telehealth platform that connects patients with licensed, board-certified physicians for disability parking permit evaluations.

Our mission

Getting a disability parking permit should not require weeks of waiting, multiple in-person appointments, or navigating confusing paperwork. Forward Health makes the process straightforward: complete an online intake, connect with a licensed physician in your state, and receive your physician-signed DMV forms within 24 hours.

Who we are

Forward Health is owned and operated by Growth Engineer LLC, a healthcare technology company registered in Washington, D.C. We work with a network of board-certified physicians licensed in all 50 states who specialize in evaluating mobility-related conditions.

Forward Health is not affiliated with Forward Health LLC (Tennessee), Forward Health Group (Wisconsin), or any other entity using the Forward Health name.

How it works

Our physicians review your medical intake and complete the official DMV certification forms required in your state. These forms are signed by a licensed physician and delivered to you electronically within 24 hours. You then submit those forms to your local DMV to receive your disability parking permit.

We do not issue placards or permits directly. The DMV issues your placard after reviewing the physician-signed documentation we provide.

Our physicians
