Forward Health (operated by Growth Engineer LLC) stands behind the quality of our physician evaluations. This Refund Policy explains when and how refunds are issued.
Money-back guarantee
If you complete your evaluation and the reviewing physician determines that your condition does not meet the medical criteria for a disability parking permit in your state, you will receive a full refund of your evaluation fee. No questions asked.
What qualifies for a refund
- The evaluating physician determines you do not qualify for a disability parking permit based on the medical criteria in your state
- A technical error prevented your evaluation from being completed and we are unable to reschedule within a reasonable timeframe
What does not qualify for a refund
- You qualify for and receive your signed physician certification forms
- Your state DMV declines your permit application after you submit our forms (DMV approval is at their sole discretion and outside our control)
- You change your mind after your evaluation is completed and forms are delivered
- You submitted inaccurate or incomplete medical information that affected the physician's evaluation
How to request a refund
Email support@getforwardhealth.com with your name and the email address used at checkout. Refund requests must be submitted within 30 days of form delivery. We will process qualifying refunds within 5-7 business days. Refunds are issued to the original payment method.
Medical records following a refund
Following a refund, your medical intake information and evaluation records are retained in accordance with applicable healthcare recordkeeping requirements. For data questions following a refund, contact support@getforwardhealth.com.
