    Eligibility

    Who qualifies for a disability parking permit?+

    You may qualify if you have a condition that significantly limits your ability to walk. Common qualifying conditions include difficulty walking 200 feet without resting, use of a wheelchair, walker, cane, or crutches, heart or lung conditions, arthritis or joint disorders, neurological conditions such as MS or Parkinson's, post-surgery or injury recovery, chronic pain that limits mobility, and limb differences or amputation. A licensed physician will evaluate your specific situation.

    Can I get an evaluation for a family member?+

    Yes. You may complete an intake on behalf of a spouse, parent, or dependent in your care. You will be asked to confirm your relationship and authority to do so during the intake process.

    Does my condition need to be permanent?+

    No. Both temporary and permanent permits are available. Your physician will recommend the appropriate type based on your condition. Temporary permits are typically issued for conditions expected to improve with treatment or recovery.

    The evaluation

    What happens during my evaluation?+

    You complete an online medical intake describing your condition, mobility limitations, and relevant medical history. A licensed physician in your state reviews your intake and determines whether your condition meets the medical criteria for a disability parking permit. If you qualify, the physician signs the official DMV certification form for your state.

    Do I need to be on a video call with the physician?+

    In most states, our asynchronous telehealth model allows the physician to review your intake without a live video call. Some states may require a synchronous visit; we will notify you if that applies in your state.

    How long does the evaluation take?+

    Most evaluations are completed and forms are delivered within 24 hours of your intake submission. Many patients receive their forms within a few hours.

    What if the physician needs more information?+

    If the reviewing physician needs clarification or additional documentation, we will contact you by email. This may extend your turnaround time slightly.

    Forms and the DMV

    What forms will I receive?+

    You will receive the official state-specific physician certification form required by your state's DMV, signed by a licensed physician. This is the document your DMV needs to issue your disability parking permit.

    Will the DMV accept these forms?+

    Yes. Our physicians complete the official DMV-required forms for your state. The forms are signed by a licensed, state-credentialed physician and are accepted at DMVs nationwide.

    Does Forward Health guarantee I will get a permit?+

    We guarantee that if you qualify medically, you will receive properly signed physician certification forms. The final decision to issue a permit rests with your state DMV. In practice, DMVs issue permits when valid physician certification is presented, but we cannot make representations on behalf of the DMV.

    How do I submit the forms to the DMV?+

    Processes vary by state. Most states allow you to submit the forms by mail, in person, or online. We include state-specific instructions with your forms.

    Pricing and refunds

    How much does it cost?+

    The evaluation fee is $129. This is a one-time payment that includes the physician evaluation, all signed DMV forms, and state-specific submission instructions. There are no subscriptions or hidden fees.

    What is your refund policy?+

    If the physician determines you do not qualify after your evaluation, you receive a full refund. See our Refund Policy for full details.

    Does insurance cover this?+

    Our evaluation is not currently billable to insurance. The $129 fee is an out-of-pocket expense.
