Telehealth Consent

This Telehealth Informed Consent document explains the nature of telehealth services provided through Forward Health (operated by Growth Engineer LLC) and your rights as a patient.
1. What is telehealth?
Telehealth is the delivery of healthcare services using electronic information and telecommunications technologies, including video conferencing, asynchronous messaging, and electronic health records. Your Forward Health evaluation is a telehealth encounter conducted by a licensed physician.
2. Scope of the evaluation
The telehealth evaluation provided through Forward Health is limited to assessing your eligibility for a disability parking permit under the laws of your state. The physician will review your medical intake, may request additional information, and will determine whether your condition meets the applicable medical criteria. This evaluation does not constitute an ongoing treatment relationship.
3. Potential benefits
- Convenient access to a licensed physician without an in-person office visit
- Same-day or next-day evaluation in most cases
- Physician-signed forms delivered electronically within 24 hours of evaluation
4. Potential risks and limitations
- The physician may determine that your condition does not meet the criteria for a disability parking permit
- Technical difficulties may interrupt or delay the evaluation
- Telehealth evaluation has limitations compared to an in-person examination; the physician cannot conduct a physical examination
- In some states, specific in-person requirements may apply for certain permit categories
5. Privacy and security
We use industry-standard security measures to protect your health information transmitted through our platform. All communications between you and our platform are encrypted. Your medical information is shared only with the evaluating physician and service providers necessary to deliver the Service, as described in our Privacy Policy.
6. Your rights as a patient
- You have the right to withhold or withdraw consent at any time without affecting your right to future care
- You have the right to inspect and copy information about yourself in our records
- You have the right to request that a correction be made to your health information
- You have the right to receive a refund if you do not qualify, per our Refund Policy
7. Emergency services
Telehealth is not appropriate for medical emergencies. If you are experiencing a life-threatening or urgent medical emergency, call 911 or go to your nearest emergency room immediately.
8. State-specific regulations
Telehealth services are subject to state-specific regulations. The evaluating physician will comply with all applicable state telehealth laws.
9. Consent
By proceeding with your evaluation on the Forward Health platform, you acknowledge that you have read and understood this Telehealth Consent, and you consent to receiving telehealth services as described herein.
